Africa’s Clinical Trial Gap: Why Stronger Regulation Could Accelerate Access to New Medicines


Africa carries almost one-quarter of the world's disease burden, yet the continent accounts for only about 2% of global clinical trials and less than 2% of global health research funding.

A new commentary in Nature Medicine argues that closing this gap will require more than additional research funding—it will require stronger, better coordinated systems for clinical-trial ethics and regulatory oversight.

This issue is particularly important for Africa's pharmaceutical industry.

Clinical trials are not simply a research activity. They are a critical part of the pathway through which new medicines are developed, evaluated, registered and eventually brought to patients.

The Nature Medicine article, published by Kara L. Neil and Jeanine Condo examines lessons from the TRACE (Trial Regulation and Clinical Ethics Optimization) initiative, which is working across Rwanda, Tanzania, Nigeria, Zimbabwe and Kenya to strengthen clinical-trial regulation and ethics review. The problem goes beyond funding

Africa's relatively small share of global clinical research is striking when compared with its disease burden.

For pharmaceutical companies developing new medicines, vaccines and technologies, the ability to conduct high-quality clinical trials efficiently is increasingly important. Delays or fragmentation in ethics and regulatory review can make African countries less attractive destinations for clinical research and potentially delay access to products once they become available.

The TRACE experience points to an important structural issue: regulatory and ethics-review systems do not always operate as a coordinated process.

According to the Nature Medicine authors, the fastest measurable improvements in trial readiness across the five participating countries came not simply from adding technical capacity, but from coordinating ethics and regulatory review through parallel processes. (Nature)

This finding has implications well beyond clinical research institutions.

Why this matters to Africa's pharmaceutical industry

A stronger clinical-trial ecosystem could make African markets more attractive to pharmaceutical companies and biotechnology firms seeking to generate evidence for new products.

For the industry, efficient clinical-trial systems can potentially contribute to:

  • faster evaluation of innovative medicines;
  • greater participation of African patients in global drug development;
  • generation of evidence relevant to African populations;
  • stronger regulatory capacity;
  • earlier engagement between pharmaceutical companies and African regulators;
  • improved preparedness for emerging diseases;
  • greater opportunities for local researchers and institutions; and
  • stronger foundations for pharmaceutical innovation on the continent.

The involvement of national regulatory authorities and ethics institutions is therefore not merely an administrative issue. It is part of the infrastructure required to support a modern pharmaceutical ecosystem.

From research participant to pharmaceutical development partner

For decades, much of Africa's role in the global pharmaceutical ecosystem has been concentrated around medicine consumption and procurement.

The continent remains heavily dependent on imported medicines, while many innovative products are developed and tested elsewhere.

A stronger clinical research ecosystem could begin to change that model.

Rather than viewing Africa primarily as a market for medicines developed elsewhere, pharmaceutical companies and governments could increasingly view the continent as a partner in medicine development.

This distinction matters.

Clinical trials conducted in African populations can generate evidence about how medicines perform in populations that are often underrepresented in global research. They can also create opportunities for African scientists, research institutions, regulators and healthcare systems to participate earlier in the pharmaceutical innovation cycle.

Regulatory convergence could become a competitive advantage

One of the biggest opportunities may lie beyond individual countries.

Africa's pharmaceutical market is highly fragmented, with different national regulatory systems, ethics requirements and approval processes.

The creation and operationalization of the African Medicines Agency (AMA) is intended to support greater regulatory collaboration and convergence across the continent.

Clinical-trial regulation could become another important area for regional cooperation.

If African countries can progressively harmonize requirements, share regulatory expertise and reduce unnecessary duplication, pharmaceutical companies could potentially conduct multi-country studies more efficiently.

For the industry, this could make Africa a more attractive destination for clinical research.

For regulators, it could increase access to expertise and improve oversight.

And for patients, it could mean greater opportunities to participate in trials evaluating potentially important medicines.

The opportunity for African pharmaceutical companies

The discussion should not be limited to multinational pharmaceutical companies.

African pharmaceutical manufacturers and biotechnology companies could also benefit from stronger clinical research infrastructure.

As local companies move beyond manufacturing established generic medicines and begin exploring biosimilars, novel formulations, vaccines and other innovative products, the ability to conduct robust clinical research will become increasingly important.

A stronger clinical-trial ecosystem could therefore support the evolution of Africa's pharmaceutical industry from predominantly manufacturing and distribution toward research, development and innovation.
